First time visit and appointment
Your first appointment at Dynamic Physical Therapy is approximately 60-75 minutes. Your therapist will let you know how long your follow up appointments will be after this initial consultation and evaluation. Download the Complete Registration Packet that should be filled out completely before your first visit. For all appointments, please wear loose fitting and comfortable clothing for treatment. Refrain from wearing a turtleneck style top. Your therapist will discuss how much work and healing your body needs. He/she will determine the best plan for follow-up time and frequency. In some cases, a shorter or longer session may be necessary. Allow this time in your schedule to assist us in your complete healing.
Payment
Whether you will be paying out of pocket for your visits or using insurance, we have plans to meet your individual financial needs. If you will be using insurance, we verify your physical therapy benefits for you as a courtesy and communicate this information prior to beginning therapy. You will receive a copy of the benefits as quoted from your insurance company to us. Please note that insurance quotes are not a guarantee of payment but we are happy to work with you to make payments in a time that fits in your budget.
Prior to your first appointment, you can download, print and fill out the complete registration packet. Please bring a valid drivers license or state id and your insurance card so that we may make copies for your records and verify your insurance benefits.
